Primary hypothyroidism

Elevated TSH with low free T4 indicates primary hypothyroidism. Most commonly Hashimoto thyroiditis; also post-radiation, post-thyroidectomy, iodine deficiency, or drug-induced (lithium, amiodarone).

Considerations

  • Anti-TPO antibodies confirm Hashimoto when clinical suspicion is high.
  • Levothyroxine 1.6 mcg/kg/day (typical replacement dose); start lower in elderly or known CAD.
