Primary hypothyroidism
Elevated TSH with low free T4 indicates primary hypothyroidism. Most commonly Hashimoto thyroiditis; also post-radiation, post-thyroidectomy, iodine deficiency, or drug-induced (lithium, amiodarone).
Considerations
- Anti-TPO antibodies confirm Hashimoto when clinical suspicion is high.
- Levothyroxine 1.6 mcg/kg/day (typical replacement dose); start lower in elderly or known CAD.
| Pattern | TSH | Free T4 |
|---|---|---|
| Primary hypothyroid | ↑ | ↓ |
| Subclinical hypothyroid | ↑ | N |
| Primary hyperthyroid | ↓ | ↑ |
| Subclinical hyperthyroid | ↓ | N |
| Central hypothyroid | N or ↓ | ↓ |
| Sick euthyroid | ↓ / N / ↑ (transient) | N or ↓ |
