Pre-op hold times for apixaban, rivaroxaban, edoxaban, and dabigatran by CrCl and bleeding risk, with restart guidance.

Last dose timing

Take the last DOAC dose 24 hours before the procedure (or earlier). Plan accordingly: a once-daily DOAC at a 24-hour hold means skipping 1 dose; at 48 hours, skip 2.

No bridging needed for DOACs

Routine bridging anticoagulation (LMWH) is not recommended for patients on DOACs around elective procedures. Their pharmacokinetics are predictable and shorter-acting than warfarin. Bridging adds bleeding risk without clear benefit in this population.
